Fill in your plan and get a plain-English service agreement with the 3-day right to cancel.
SERVICE AGREEMENT
Lakeshore Pest Solutions ("we") will provide general pest control (inside and out) to [customer name] ("you") at [service address], every 3 months.
PRICE. The first service is $149. Each regular service after that is $129. Prices may rise once a year; we will tell you at least 30 days before.
TERM. This agreement runs for 12 months and renews month to month after that. Either of us may cancel with 30 days' notice. If you cancel before the first 12 months are up, a $75 early cancellation fee applies.
RE-SERVICE. If pests covered by this agreement come back between regular visits, we'll come back at no charge.
YOUR RIGHT TO CANCEL. You may cancel this agreement without penalty within 3 business days of signing by telling us in writing.
PAYMENT. Payment is due at each service unless you choose autopay. Accounts more than 30 days past due may have service paused until paid.
PRODUCTS AND NOTICES. We use EPA-registered products according to their labels. You'll receive the notices WI requires after each application, by email if you agree in writing.
